Predicting the Outcome of Prostatectomy Using Noninvasive Bladder Pressure and Urine Flow Measurements.

The CT3000 is a non-invasive bladder pressure measurement technique involving controlled inflation during voiding of a cuff placed around the penis. The cuff pressure at which flow is interrupted (pcuff.int) provides a valid and reproducible estimate of isovolumetric bladder pressure (pves.isv) - a measure of detrusor contraction strength. The CT3000 produces a plot from which maximum values of pcuff.int and maximum urinary urine flow rate (Qmax.cuff) can be read. These measurements can then be used on a pressure flow nomogram to categorise obstruction. Patients classified as having Bladder Outlet Obstruction (BOO) with the CT3000 have an 87% chance of a good outcome from TURP.
Treatment options for men with lower urinary tract symptoms (LUTS) due to benign prostatic enlargement have increased over the last 20 years, although surgical removal of tissue typically by transurethral resection (TURP) remains most effective. This choice has focused the need for more precise diagnostic tests that can predict outcome and hence guide treatment selection.
Men with BOO defined by invasive pressure flow studies (PFS) have success rates following TURP that are 15–29% higher than those without obstruction, but invasive PFS are not commonly performed because of patient discomfort, infection risk, and cost associated with the need for skilled staff and specialised equipment. Most United Kingdom urologists therefore suggest surgery on the basis of bothersome LUTS unresponsive to drug therapy and associated with reduced urinary flow, with 65 – 75% of men achieving satisfactory symptomatic benefit. The CT3000 raises this outcome success to 87%, which improves the outcome prediction for men considering surgery by 12 – 22% representing a significant cost saving as well as reducing patient morbidity.
The CT3000 is not designed to replace PFS but act as a screening tool to classify patients with LUTS. It increases the outcome of surgery for those patients that are classified using symptoms alone. This represents a significant cost saving as well an opportunity to see more patients in a shorter period of time with a reduction in disposable costs as well.
